With these plans, you have ACCESS TO ALL THE MAJOR HOSPITAL SYSTEMS IN OHIO as well as thousands of doctors and hospitals all across the country through the FIRST HEALTH PPO network!!!! You can also choose a Referenced Based Pricing (RBP ) plan that doesn’t use a network!!

HOW TO APPLY

STEP 1:

Click on the secure link to complete the Personal Health Questionnaire: PHQ
( This step is simply to determine your rating tier. You are not bound to coverage at this time )

STEP 2:

An agent from Blascak Insurance Services will reach out to you within 2-4 business days to provide you with your rating tier

STEP 3:

Check to see if your Doctor/Hospital is in the network: Provider Search

When prompted, CHOOSE THE FIRST HEALTH NETWORK OPTION, then complete the requested information to locate a provider

STEP 4:

Choose your plan by the 15th of the previous month and complete the online ACH link.

STEP 5:

IHA will draft the funds from your account on the 20th of each preceding month
